The band formed in Los Angeles and had several line-ups. The original line up in 1999 was Duff on bass and vocals, Dez Cedena (founding guitarist of Black Flag) on guitar, Michael Barrigon (from Plexi) on guitar, and Taz Bentley (formerly in Reverend Horton Heat & Tenderloin) on drums.
Later Duff teamed up with drummer Geoff Reading (formerly of New American Shame), guitarist Dave Kushner (presently in Velvet Revolver) and bass player G. Stuart Dahlquist. They released the album "Episode 1999: Live" on May 27, 1999.
Later in 1999, Duff and Taz toured Japan with Izzy Stradlin. Then in 2000 a new lineup of Loaded was assembled in Seattle: Geoff Reading on drums, Duff on guitar and vocals, Dave Dederer (P.U.S.A.) on guitar, and the bassist from Alien Crime Syndicate. ACS's guitarist later replaced Dave Dederer when he reunited with P.U.S.A.
In the winter of 2000-2001 Loaded recorded their second album, "Dark Days" by Martin Feveyear. It features Duff on vocals, guitar, bass, and piano; Geoff Reading on drums; Taz Bentley plays drums on the last tune. Dave Dederer plays guitar. Martin Feveyear produced the album and also played keyboards.
When Velvet Revolver formed, Loaded was disbanded, so that Duff McKagan could focus on VR's album. They reunited for one gig in Seattle in December 2006.
